CCHI offers a Template for Documentation of Continuing Education which you may use when a course sponsor/instructor does not provide a Certificate of Attendance or CEU/CE documentation.

NOTE: Completion and submission of this template does not guarantee acceptance of the continuing education described by CCHI. CCHI will conduct an evaluation of the information provided to ensure that it meets the requirements for CCHI’s continuing education as described here.  You should keep all documentation of continuing education in your records until CCHI requests documentation.
